Cancellation Policy

We Understand "Life Happens"

At Unique Health, we understand that life is unpredictable and things happen. However, we do require a minimum of 12 hours notice to cancel or modify an appointment. Any missed, last minute cancellation, and last minute rescheduling of an appointment will now be subjected to a 100% charge of the total cost plus all applicable taxes of the scheduled appointment. Please note that direct billing your insurance company is NOT an acceptable method of payment for any missed appointments.
